Are you a health professional working with adults who need healthy lifestyle support?

The Better Health Coaching Service is a health coaching program for eligible adults in Western Australia. We are accepting health professional referrals to the service.

This program can help you to support and encourage patients to improve their habits and lead healthier lifestyles, long-term.

When you refer your patients, they get access to up to 10 x 30 minute health coach appointments. Our health coaches are tertiary qualified health professionals whose backgrounds include dietetics, exercise physiology, and psychology.

About Better Health Coaching

The Better Health Coaching Service provides one-on-one health lifestyle support to eligible adults in Western Australia

Designed by the Better Health Company’s team of nutrition, psychology and exercise professionals, the coaching service offers practical skills and knowledge that lead to sustained change.

Outcomes can include:

  • Increased consumption of fruit and vegetables
  • Healthy physical activity levels
  • Maintaining a healthy weight
  • Improved emotional wellbeing
  • Increased knowledge of health and nutrition information

How do I make a referral?

Referring to this service is as easy as completing this online referral form with your patients details. Or if you’d prefer, you can call and speak with us directly on 1300 822 953.

Once you have made a referral, the Better Health team will be in touch with your patient to get them started. There are no waitlists to join. Coaching appointments are available from 9am until 9pm Australian Western Standard Time (AWST).

A referral from a trusted health professional is often the motivation people need to take the next step towards better health. The program is designed to complement the client care you offer, and provides you with a practical solution for clients. By referring people to Better Health Coaching you can:

  • Complement your care plans. Enhance your clients care by integrating Better Health Coaching.
  • Provide expert guidance to your clients. Better Health Coaching is delivered by experienced professionals specialising in chronic disease management, including dietitians, exercise physiologists, and health psychologists.
  • Help your clients prevent or better manage preventable chronic conditions through healthy lifestyle support.
  • Empower adults to improve and maintain their health and wellbeing.
  • Receive regular updates on your client’s progress.

Once you have referred, our team will be in touch directly with the participant to get them started. With the client’s consent, you will receive updates on their progress.

Who we work with

FAQ

Adults are eligible to join for FREE if they:  

  • Are over 18 years of age.
  • Live in Western Australia.
  • Need healthy lifestyle support to prevent or manage a newly diagnosed health condition.

People who are pregnant at time of registration or referral are not eligible for this service at this time.

Our health coaches are trained to provide health lifestyle support for a range of different chronic health conditions including but not limited to:  

  • Diabetes or pre-diabetes   
  • Heart disease (e.g. high blood pressure, heart failure, blocked arteries)  
  • Stroke or nerve problems  
  • Cancer rehabilitation   
  • Musculoskeletal conditions (e.g. arthritis, osteoporosis)  
  • Respiratory conditions (e.g. Asthma, COPD)  
  • Kidney or liver conditions  
  • Weight loss  
  • Mental health   
  • Chronic pain  

This service is powered by Better Health Company, and funded by the Department of Health Western Australia.

Our health coaches are university qualified health professionals with backgrounds in nutrition / dietetics, exercise science, psychology or nursing. They are trained in chronic disease management and experienced with supporting healthy behaviour change. Typically, you’ll have the same health coach who will support you right throughout the program. 

Our health coaches provide healthy lifestyle support to compliment your care plans. While they do not diagnose health conditions, offer medical advice, or create treatment plans, they do help people implement the guidance received from their health professionals through health behaviour changes.  

Our health coaches can support your clients with:  

  • Healthy eating 
  • Getting active 
  • Mindfulness 
  • Reducing stress 
  • Building social connections 
  • Cutting down on alcohol 
  • Quitting smoking 

Any advice or recommendations provided by health coaches as part of this service aligns with national guidelines including the Australian Dietary Guideline and the Australian 24 Hour Movement Guidelines.  

Participant measurement and health data is collected in pre and post participation surveys and will typically include physical activity habits, eating habits, alongside quality-of-life survey, health literacy measures and wellbeing metrics. Questionnaires and survey tools are determined by the contract or program funder. All data is collected online via Better Health Company’s survey portal and stored in our secure data management system, which is private and confidential. 

Participants are not eligible to join if they live outside of Western Australia, are under the age of 18 years or are pregnant at time of registration.  

Yes, eligible participants who have already completed the Better Health Coaching Service can sign up again if they still require healthy lifestyle support to prevent or manage a chronic health condition and have a referral from a GP or health professional.
