To ensure your referral is eligible for the service our friendly team will give them a call soon and confirm their place.

Who is eligible?

Adults are eligible to join if they:

• Are over 18 years of age.
• Live in Western Australia.
• Need healthy lifestyle support to prevent or manage a newly diagnosed health condition.
